Oral Consent
Oral consent scripts should be written in language you would be comfortable using – language that comes naturally to you.
The Consent Form Outline may be used to organize your thoughts. The script should not include contact information unless you plan to give the participants a copy of the script. Cards may be used to provide contact information.
Foreseeable risks of harm and potential benefits to participants only need to be addressed if they exist.
